Move to Madeira
Registering for healthcare on an island where the service is SESARAM

The short answer
Public healthcare on Madeira is delivered by SESARAM, the Serviço de Saúde da Região Autónoma da Madeira, not by a mainland ARS. Registration is made in person at the centro de saúde for the area of residence, or at the Balcão do Cidadão at Hospital Dr. Nélio Mendonça in Funchal, and produces a número de utente.
The mainland structure — regional health administrations, the SNS portal, the mainland front doors — is not what a person actually deals with here. Public healthcare on Madeira is delivered by SESARAM, EPERAM, the Serviço de Saúde da Região Autónoma da Madeira, and the counterparty at every stage is regional.
The confusion is understandable, because the layer underneath genuinely is national. Getting the two apart is the whole of this page.
The service is regional. The register behind it is national
The substantive requirements and the underlying Registo Nacional de Utentes are national. The difference is organisational: the counterparty is SESARAM and the regional health centres, and the digital front end is the SESARAM Portal do Utente rather than the mainland SNS portal.
So a guide that says "register with the SNS" is not wrong about the entitlement. It is wrong about the building, the website and the name of the body that answers. For someone standing in Funchal with a folder of documents, that is the part that matters.
Where registration is made
Registration is made in person. There are two routes: the centro de saúde for the area of residence, or the Balcão do Cidadão at Hospital Dr. Nélio Mendonça in Funchal.
The catchment matters. The health centre is tied to the address, which means the registration is one of the first things that fixes a person to a specific part of the island rather than to Madeira in general.
What the health centre asks for
The documents reported for registration are:
- an identity document, a passport or a Cartão de Cidadão;
- proof of the right to reside: the Certificado de Registo de Cidadão da União Europeia for EU citizens, or the residence permit card for third-country nationals;
- a Portuguese NIF;
- proof of address in Madeira, within the catchment area of the health centre, a rental contract or a utility bill.
Two of those four have to be obtained first and neither is instant, which is why the registration certificate an EU citizen gets from the câmara municipal and the tax number sit upstream of this step rather than beside it.
The número de utente, and what it does not produce on its own
Registration creates the person's record in the Registo Nacional de Utentes and produces a número de utente. After that the person may be assigned a médico de família or placed on the waiting list for one, and may register on the SESARAM Portal do Utente.
The number and the family doctor are two different things, and the gap between them is where new arrivals are most often surprised. This page publishes no waiting time for that assignment, because no sourced figure for it was obtained. A waiting list with an invented number attached to it is worse than a waiting list.
The acute hospital is in Funchal, and that is a location decision
Hospital Dr. Nélio Mendonça, in Funchal, is the hospital named in SESARAM's own registration route. The island's acute capacity is concentrated on the south coast, in the city.
A house on the north coast is a mountain crossing away from it. In good weather that is a drive. In bad weather, at night, with something time-critical happening, it is a different proposition, and it is a health decision wearing the costume of a view. Anyone weighing the north against the south should read how the towns actually compare with the hospital on the map rather than off it.
What this page could not source
No SESARAM regulation or regional diploma governing utente registration was located at a primary source for this guide. The document list above reflects SESARAM and national guidance as reported, not a legal instrument read directly.
That is a real limitation and it is stated here rather than smoothed over. The organisational fact, that the service is SESARAM and not a mainland ARS, is solid and is what the rest of the internet gets wrong. The precise documentary requirements are the part that a health centre could revise without anyone publishing a diploma about it.
Questions people actually ask
Is healthcare in Madeira part of the SNS?
The entitlement and the patient register are national — registration creates a record in the Registo Nacional de Utentes. The service itself is delivered by SESARAM, the regional health service, through regional health centres and the SESARAM Portal do Utente rather than the mainland SNS portal.
What is a número de utente?
It is the patient number generated when a person is registered in the Registo Nacional de Utentes. It is what identifies the person inside the public health system, and it is produced by registering at the centro de saúde for the area of residence or at the Balcão do Cidadão at Hospital Dr. Nélio Mendonça.
Which documents does the centro de saúde ask for?
An identity document, proof of the right to reside, a CRUE for an EU citizen or a residence permit card for a third-country national, a Portuguese NIF, and proof of an address inside the health centre's catchment area, such as a rental contract or a utility bill.
Does registering mean getting a family doctor straight away?
Not necessarily. Registration produces the número de utente; a médico de família may then be assigned, or the person may be placed on a waiting list. This page publishes no waiting time, because no sourced figure for it was obtained.
Where is the main hospital on Madeira?
Hospital Dr. Nélio Mendonça, in Funchal, the hospital named in SESARAM's own registration route. Acute capacity being concentrated in the city is a live consideration for anyone weighing a house on the north coast, where the journey crosses the mountains.
